Housing Applications Housing Applications are available at the Housing Authority Housing Department in the lower level of 1040 Myrtle Avenue or at any property leasing office. Any additional Adult over the age of 18 in your house hold must complete and sign a separate Additional Adult Application Instructions for completing an application Complete the application and take it to 1040 Myrtle Avenue or mail it to P.O. Box 1029, Jefferson City, MO 65102. Postage for a legal size envelope will be 78 Cents; Postage for a 9 by 11 envelope will be $1.22. COMPLETE AND RETURN ALL 18 PAGES OF THE APPLICATION. APPLICATIONS THAT ARE INCOMPLETE OR CANNOT BE READ CANNOT BE PROCESSED. WE DO NOT ACCEPT FAXED APPLICATIONS. IF YOU FAX THE APPLICATION, WE WILL NOT ACCEPT, PROCESS OR KEEP IT. YOU MUST SEND THE ORIGINAL SIGNED APPLICATION. |
